*  In order to accurately complete your FHA Fee Inspection, we will need for you to submit copies of the following so that we may verify the content of the file.  If the information needs to be returned, a self-addressed, stamped/postage-paid envelope should also be included.  Otherwise, all documents will stay on file with the completed HUD 92051 form at our office.  Please fill out the form below to submit all necessary documents & information in order that we may complete your FHA Final Fee Inspection.        

       

(BLACK highlighted documents MUST be sent to us in order that we have specifications by which to do our inspection.  For RED high-lighted items below, this sheet may be printed out, information written or typed in & sent to us instead of sending whole documents.)

 

If we do not receive confirmation that the below documentation is either on file with you or if it is not included in our package from you, we will note those item as "not included" on the HUD 92051 & they will need to be included in the FHA case file before filing/completion.

Part 1.   Items Necessary to fill out HUD 92051 Report (in red):

 

1.  The name of the 10-Year Warranty Company noted in the Letter of Conditional       

     Commitment as evidence that the 10-Year Warranty is in effect;  

               

 

        2.  The FHA Case Number; 

               

 

        3.  The Full Builder's Name & Contact Information of the corporate office  

               

 

             AND

           

            the subdivision/builder contact information for the current case home in order that we

            may gain access to perform our inspection;

               

 

        4.  the FULL Legal Description of Property & the full name of the buyer;

               

 

        5.  Mortgagee's Full Name & Contact Information; 

               

 

 

Part 2.    Required drawings to perform inspection:

 

        6.  Required Drawings (full set):

                a.  Engineered Roof Truss Design;

                b.  Plot Plan;

                c.  Plot Plan showing flood/grading & drainage swales;

                d.  Street Plans;

                e.  Floor Plans;

                f.   Engineered Foundation Design Drawings;

                h.  Building Sections;

                i.   Electrical Layout Drawings;

        7.  The Foundation Post-Tension Cable Stress Letter Signed by the presiding engineer;  

               

 

        8.  The Final Grade Certification signed by the presiding superintendent;

               

 

        9.  Either the HVAC Heat Loss Rating Calculations (Schedule J - 1992 Model Energy Code Work

            Data) or a letter of proof that the document is on file with the lender;

               

 

        10.  HUD 92541 - Builder's Certification completed & signed by the builder;

               

 

        11.  HUD 92541-A  - Executed Builder's Warranty of Completion of Construction; 

               

 

        12.  HUD 92544 (or HUD 92544-A whichever is applicable) Warranty of Completion of

               Construction; 

               

 

        13.  HUD 92005 - Description of Materials; 

               

                 

        14.  URAR (Appraisal Report) (if available) &/or HUD 92800.5B Conditional Commitment /Direct

               Endorsement Statement of Appraised Value; 

               

 

        15.  HUD 92577 (if applicable)

               

 

        16.  HUD NPCA-99-A - Termite Report/Soils Report; 

               

   

        17.  Well &/or Septic Permits (if applicable) & information including bacteriological tests&/or

Assuming all required documentation has been properly submitted to us in order that we can do our inspection, the report will be completed by open of business the following day.  Upon completion of the inspection, a copy of the report will be made available to you via our website.  The file will be saved in *.PDF format & you may print as many copies of the form as you need.  The pictures will also be included.
